Million Dollar Smiles 2012 Bearhug Program

OPACC is collecting registrations on behalf of the Million Dollar Smiles 2012 Bearhug Program. Million Dollar Smiles is a non-profit organization which exists to put smiles on faces of children facing major challenges in their everyday lives. The Bearhug Program distributes free 4' tall teddy bears and $100 Toys R Us gift cards. You can ask for a form at clinic, at the Tuesday night drop-ins, or print out the form here and either scan and email or fax it back to Million Dollar Smiles at the contact information at the bottom of the form.

If you don't have access to a scanner or fax, you can also send us your information to info@opacc.org and we will pass it along to Million Dollar Smiles. Please include the following information:
